Cant help with the sleeping. As for waking up, the solution is simple: alarm clock.

What time do you go to sleep, and what time should you wake up?

school ends at 12noon[we got double session(70 minutes a class!!!)], last year ended at 1pm, cuz most high schoolers got part time jobs already.

for falling asleep, try to clear your mind and relax. and for waking up, try (if not already) showering in the morning instead of at night, or get a very loud alarm clock that scares you when it goes off, then your heart’s pounding and harder to fall back to sleep

You should go to the doctors . There are people in my family that had that and it turned out to be something serious . Diabetes , lack of right vitamins , something like that . To be on the safe side it would be a good idea to go to the doctors .
